Dr. P. Daniel Ward is a double board-certified facial plastic surgeon and the founder of Ward MD Facial Plastic Surgery, Form Surgery Center and Form Med Spa in Salt Lake City. He began his career on faculty at the University of Utah in 2010 and left in 2016 to build his own practice, where he continues to teach as an adjunct associate professor. His surgical focus is rhinoplasty, revision rhinoplasty, facelift, neck lift and blepharoplasty, with a physician-led hair restoration program.
